REVIVE Chiropractic Five Points Business Park
Hello! We are located at the Five Points Business Park on the West side of N Saginaw Blvd / Business 287. You will see "Fitness Fight Factory" when you pull into the entrance. Take a right and drive around their building. REVIVE Chiropractic is located in the second building on the left, inside REVIVE Wellness + Aesthetics. When you enter the door, please scan the QR code at the front desk to check in. If you are a new patient, there will be a clipboard with chiropractic intake forms for you to fill out.
